Dear College Club Team Coaches and Organizers,
Club Teams must confirm their intent to participate in the 2013 CSA Men’s and Women’s Team Championships by January 15th, 2013.
All club teams planning on attending this year’s end of season CSA Team Championships should fill out the below form in order to confirm their registration. Teams not completing the form may not be eligible to compete. As noted on the form, if you represent both the Men’s and Women’s Teams at your institution, please fill out the form once independently for each team.

As a reminder, by CSA rule all club teams must meet the following requirements in order to be eligible to compete in the team championships:
“Club teams are required to play eight (8) matches against other CSA teams to enter team championships. In addition, club teams must play at least four (4) matches between January 1st and February 10th. At minimum, each team should have at least five (5) different opponents.”
Teams must have also paid their CSA membership dues and tournament entry fees in order to attend the event.
